Prognostic significance of microvascular dysfunction

چکیده

Several studies have demonstrated a strong association between coronary vascular endothelial dysfunction and an adverse long-term cardiovascular prognosis. Both conduit arterial and microcirculatory endothelial function are predictive of outcome, independent of the presence of coronary artery disease and its risk factors. Thus assessment of endothelial function or its markers in patients with different cardiovascular disorders may help identify a subgroup of patients at high risk. Whether strategies that improve microvascular function will uniformly improve prognosis needs to be studied prospectively. Heart Metab. 2008;40:11–14.

Prognostic Significance of Peripheral Microvascular Endothelial Dysfunction in Heart Failure With Reduced Left Ventricular Ejection Fraction.

BACKGROUND Endothelial dysfunction plays a crucial role in heart failure (HF), but the association between peripheral microvascular endothelial function assessed by reactive hyperemia peripheral arterial tonometry (RH-PAT) and prognosis remains unknown in HF with reduced left ventricular (LV) ejection fraction (HFREF). We prospectively investigated the association between peripheral microvascul...

Coronary microvascular function, myocardial metabolism, and energetics in hypertrophic cardiomyopathy: insights from positron emission tomography.

Hypertrophic cardiomyopathy (HCM) is a major cause of sudden cardiac death in adolescence, and may lead to heart failure at any age. However, significant heterogeneity in the clinical course and phenotypic expression exists. Next to left ventricular hypertrophy, an impaired myocardial blood flow (MBF) during stress and inefficient cardiac metabolism are other characteristics of HCM. Studies usi...

Thermal provocation to evaluate microvascular reactivity in human skin.

With increased interest in predictive medicine, development of a relatively noninvasive technique that can improve prediction of major clinical outcomes has gained considerable attention. Current tests that are the target of critical evaluation, such as flow-mediated vasodilation of the brachial artery and pulse-wave velocity, are specific to the larger conduit vessels. However, evidence is mou...

Coronary microvascular dysfunction in the clinical setting: from mystery to reality.

Far more extensive than the epicardial coronary vasculature that can be visualized angiographically is the coronary microcirculation, which foregoes routine imaging. Probably due to the lack of techniques able to provide tangible evidence of its crucial role, the clinical importance of coronary microvascular dysfunction is not fully appreciated. However, evidence gathered over the last several ...
